Issue: S-hook gap exceeds 0.04 in. — an entanglement and chain-separation hazard.

Recommended Action: Remove swing from service. Replace or close S-hook with manufacturer-approved hardware.

Exposed rebar spike at border timber

Issue: Rotted timber has exposed a protruding rebar stake inside the use zone — impalement and tripping hazard.

Recommended Action: Cut rebar flush or remove; replace deteriorated border section.

Displaced surfacing below required depth

Issue: EWF measured at 4 in. at slide exit; 9–12 in. is required for the 6 ft fall height.

Recommended Action: Top off and rake surfacing to 12 in.; consider wear mats at high-traffic exits.

Issue: Bolt extends beyond nut by more than two threads, failing the protrusion gauge test.

Recommended Action: Trim bolt and install manufacturer cap nut.
